Insurance Patients

Let Dr. Liu help you obtain the maximum benefits that your plan provides. Her office will gladly process claims for most major insurance plans. You are also able to receive an estimate for the portion of your bill that is not covered by insurance. This co-payment amount is due at the time of treatment and may be paid by any of the options listed above.

Estimates are based on the information that you provide regarding the benefits of your company's insurance plan. Dr. Liu's office is not party to your insurance contract and therefore cannot guarantee what your insurance will cover. Any balance not paid within 30 days due to insurance company delay or because forms were not provided at the time of treatment will be your responsibility.

Dr. Liu delivers the finest care at the most reasonable cost to her patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

Cancellation Fees

You are able to cancel or reschedule your appointment if necessary. However, Dr. Liu requests that you give us 24-hour notice to avoid a cancellation fee.
